The Middlefield Recreation Department is taking registration for Flag Football, Tackle Football, Cheerleading, Fall Soccer and Volleyball with the fee deferred until the start of the program.
Effective August 1, 2017
In accordance with Lindsay’s Law, Village of Middlefield Recreation Department is required by the State of Ohio to collect signed forms from all parents and athletes. Your coaches and/or program leaders will present you with these forms at the start of your session.

THE ONLY PLAYER OR PLAYERS GUARANTEED TO BE ON A SPECIFIC TEAM IS/ARE THE COACH’S CHILDREN. WE NO LONGER ACCEPT REQUESTS FOR A COACH, TEAM, OR ANY OTHER PLAYER FOR ANY REASON!
If you can’t make our office hours of Monday – Friday 8 a.m. – 5 p.m., print and fill out a registration form and mail it to us or drop it in our payment box located on the north side of the municipal building, before the registration deadline.Registration, Cancellation & Refund Policies
All programs must meet class minimums and maximums in order to be conducted. MRD reserves the right to cancel, combines, change time or date, or make any other revisions (within 2 weekdays of the programs beginning date) which may be necessary. ALL fees must be paid in full at the time of registration. Registration cannot be accepted if the payment is incorrect or does not accompany registration. Participants living outside of Middlefield Village will be charged an additional fee of $10 for all programs. This charge is reflected in the non-resident (NR) fee listed with each program in this brochure. All participants working inside the Village are considered Corporate Resident’s and will be charged the regular resident rate for programs (a recent pay stub showing the participant’s name and business address must be provided at registration).A $20 late fee will be charged to all participants who register after the registration deadline.
You may only register for your self or as a legal guardian of your family (aunts, uncles, grandparents, etc may not register participants for programs).
Participants registering for age specific programs must be the minimum age or grade listed by the first day of the program unless otherwise stated.
FAMILY DISCOUNT PROGRAM
First child (highest priced program) full price of program.
Second child (next highest priced program) 15% off of price of program.
Third child (next highest priced program) 20% off of price of program.
Fourth and up (next highest priced program) 25% off price of program.
Must register in office to receive discounts.
If a class is cancelled by MRD, you will receive a full refund in the form of a Village check within four to six weeks, according to the Finance Department’s bill-paying schedule. There will be no cash refunds.
Refund Policies A $15 administrative fee will be charged for ALL refunds to cover processing costs for each individual enrollment. Additional charges may be deducted to cover expenses that apply to the program (t-shirts, supplies, etc).All refunds will be made according to the following guidelines:Refunds Before the Class Begins. Refunds will be made only before the start of the class/program for the following circumstances:When the refund is requested at least five (5) business days in advance of the first class meeting date. When a participant becomes ill (you must present a doctor’s statement). Refunds After the Class Begins A Village check will be issued after the class begins only if the participant becomes ill (you must present a doctor’s statement).
Program Transfers Program transfers must be requested at least five (5) business days in advance of the first class/program meeting date. All program transfers will be assessed a $5 administration fee as well as any other additional fess that may apply to the program (t-shirts, supplies, etc).
Photographs are occasionally taken of program participants during various programs. These photographs are used to promote Middlefield Recreation programs in future editions of Activities Guide and a variety of other Middlefield Recreation publication. If you prefer that you or your child not be photographed, please notify us.
Inclement Weather Cancellations
In case of sever weather, please call the MRD’s announcement line a 440-632-5823 regarding weather-related program and class cancellations. The announcement line is updated only when there are cancellations.
The Village of Middlefield offers financial assistance for Cardinal School District residents through the Nina Marcu Private Leisure Assistance for Youth (P.L.A.Y) trust fund. Assistance can be used for participation in Youth Sports programs offered by Middlefield Recreation Department. Contact the MRD for more info.
Donate to the Nina Marcu P.L.A.Y Trust Today!
Donations of all levels are greatly appreciated and are tax deductible. Monies donated to the Nina Marcu P.L.A.Y Trust Fund are used to enroll Cardinal School District youth, who would not be able to afford it otherwise, in our many recreational activities and programs. For example, a donation of $40 could send a child to football camp.
