In accordance with Lindsay’s Law, Middlefield-Cardinal Youth Sports Program is required by the State of Ohio to collect signed forms from all parents and athletes.  These  forms are included with the registration application.

THE ONLY PLAYER OR PLAYERS GUARANTEED TO BE ON A SPECIFIC TEAM IS/ARE THE COACH’S CHILDREN.  WE WILL TRY TO ACCEPT REQUESTS FOR A COACH, TEAM, OR ANY OTHER PLAYER HOWEVER IT IS NOT GUARANTEED.

Funding Assistance

There is  financial assistance for participants with need through the Domen/Marcu Foundation. The  Geauga Department of Job and Family Services has a program for families in need as well to assist with  participation fee payment.  Contact sara.shininger@jfs.ohio.gov  or call 440-285-9141, ext.1263 to fill out an application for Assistance

The Nina Marcu P.L.A.Y trust fund is now, The Domen / Marcu Foundation.

Donations of all levels are greatly appreciated and are tax deductible.

How to apply:

2. Return completed application, with proof of eligibility, to the Middlefield Municipal Center or mail it to the address at the bottom of this page. * All applications must be submitted at least 10 business days prior to the registration deadline of any program.

Inclement Weather Cancellations

In case of sever weather, your team’s coach will notify you of weather-related cancellations.

Family Discount Program

First child (highest priced program) full price of program.
Second child (next highest priced program) 15% off of price of program.
Third child (next highest priced program) 20% off of price of program.
Fourth and up (next highest priced program)  25% off price of program.

Photo Release

Photographs are occasionally taken of program participants during various programs. These photographs are used to promote Middlefield Recreation programs in future editions of Activities Guide and a variety of other Middlefield-Cardinal Youth Sports Program publication. If you prefer that you or your child not be photographed, please notify us.

Refund Policies – Charges will be deducted to cover expenses that apply to the program (team uniform, supplies, etc). A $15 administrative fee will be charged for all refunds after start of program to cover processing costs for each individual enrollment.
