Spring 2026 Youth Sports Flyer
- Registrations can be emailed to CNaftzger@middlefieldohio.com or dropped off to 14860 N State Avenue, Middlefield, Ohio 44062.
- If after 4pm (Monday-Friday) please drop off the registrations in the night drop box located on the side of the building.
- If paying by credit card, and dropping off registration or emailing after hours please write that on the application and we will call you to make payment.
